I’ve replaced my temp sensor and it is still doing it. If I unplug the battery and reconnect everything will work just fine for a few days, and then stops working again.

You may want to check your EGR valve, I believe the cooling fan signal is tied to the EGR. I just can't remember the specifics of how they interconnect.
